4 Months of Supervised Weight Loss Recordings:

This is the plan for those of you who require either 4 months or 12 weeks of insurance-mandated supervised weight loss visits. You will receive your pre-recorded sessions via email every 30 days from when you sign up along with instructions for what to do to get credit for each session. Aetna patients will be required to send in weekly weigh-ins between courses to me to get their 12 credits.
Each course is about 20 minutes long, except the first course, which is about 43 minutes long. It is required to complete your courses 30 days apart (1 per month). At the end of each course you will fill out your name and weight for the month. This is how you will get your credit for your monthly nutrition courses.
It is very important that you attend your courses each month. If you skip a month, your insurance can use that as an excuse to disqualify you from surgery until you have done your appointments consecutively.  Do not try to do two courses in one month, because this will not count towards your 4 months (or 12 weeks) of supervised weight loss. I am available to you via email or text throughout this process, so please feel free to ask any questions you may have!
Course Value: $235 (4 monthly appointments)
or $595 (12 weekly appointments)
Savings: $135
